PSA CERTIFICATES
Civil registry documents issued by the Philippine Statistics Authority (PSA) are a basic requirement for various consular services such as passports and reacquisition of Philippine citizenship. These include birth certificates, marriage certificates, certificates of no marriage, and death certificates. They serve as official and authenticated proof of identity, citizenship, and civil status.
Please note that PSA documents must be obtained in the official security paper, which guarantees the authenticity of the certificate.
If the consular service you are applying for requires any of these PSA documents, it is crucial to secure the necessary documents before scheduling your appointment. Failure to present the required documents may result in delays or an inability to process your application, which could necessitate rescheduling your visit.
CIVIL REGISTRY REMINDER
If you have reported your birth, marriage, or death to the Embassy, the approved Report will be forwarded to Manila to become a permanent record of the PSA. Please do not forget to request the transmittal details (Reference Number, Dispatch Number, Dispatch Date and Transmittal Date) from the Department of Foreign Affairs-Consular Records Division (DFA-CRD) through oca.crd-us@dfa.gov.ph.
After six months from the date of approval of the Report, you must request an authenticated copy from the PSA through psahelpline.ph or psaserbilis.com.ph. You will need the transmittal details provided by the DFA to complete the online order.
HOW TO GET A PSA CERTIFICATE
If you live overseas, there are two ways for you to get a civil registry document (birth certificate, marriage certificate, certificate of no marriage, death certificate):
- Through an authorized representative in the Philippines
- Through PSA Serbilis or PSA Helpline
NO RECORD
When you request for a copy of a PSA document and you are given a Negative Results Certification instead, it means that the PSA could not find that certificate in its database.
If the PSA could not locate your records, one of the following situations may have occurred:
- Local Civil Registry (LCR) Has Your Record, but Failed to Endorse it to PSA
- What Happened: The LCR has your birth, marriage, or death registration, but did not forward it to the PSA for certification.
- Action Needed:
- Visit the Local Civil Registry (LCR) to request an endorsement letter for your record to be submitted to the PSA.
- Ensure you receive:
- Endorsement Letter from LCR.
- Reference Number for your request.
- Date of Delivery to know when to follow up with the PSA.
- Hospital or Midwife Failed to Register Your Record within the 30-Day Period
- What Happened: The hospital or midwife responsible for your birth registration failed to submit it to the LCR within the required 30-day period, meaning no official record was created.
- Action Needed:
- File a Late Registration of Birth (or marriage or death) at the LCR so that a new record can be created.
- Submit the necessary documents for late registration (e.g., valid IDs, affidavits, etc.).
- Your Birth Record Got Lost or Destroyed at the LCR
- What Happened: Your record was lost, possibly due to calamities like fire or flooding, or it may have been destroyed or misplaced, especially if you were born during or shortly after WWII.
- Action Needed:
- Request Reconstruction of Your Birth Certificate at the LCR
- The LCR will recreate a new copy of your birth certificate and submit it to the PSA for encoding and certification.
